II. 更新於仁孚行有限公司之登記資料 Update contact details maintained with Zung Fu Company Limited
請於適當方格加上“√” 號 Please tick the relevant boxes for the changes required
* 請提交 (i) 有效車輛登記牌簿 (正及底面) 及 (ii) 商業登記証副本（只適用於以公司名義註冊之車輛）以作核對
Please provide a copy of the (i) valid Vehicle Registration Document (front and back), and (ii) Business Registration Document (for company-owned vehicle only) for verification
* 請提交 (i) 有效車輛登記牌簿 (正及底面) 及 (ii) 商業登記証副本（只適用於以公司名義註冊之車輛）以作核對

III. 更新於安盛保險或梅賽德斯-奔馳財務服務香港有限公司之登記資料
Update contact details maintained with AXA General Insurance Hong Kong Ltd or Mercedes-Benz Financial Services Hong Kong Limited
Zung Fu will transfer the above updated personal data (“Data”) to AXA General Insurance Hong Kong Ltd or Mercedes-Benz Financial Services Hong Kong Limited (“MBFSHK”) for updating your records. We will not use your Data for other purposes and will not transfer or disclose your Data to third parties without your consent. Please ”√” the box if you agree to such transfer.
